Covid-19 Dr. Berkowitz is fully vaccinated and will wear a mask upon request.

Office Hours
The office is open at 10:00 AM to 6:00 PM on Monday and Wednesday. Visits are available by appointment only.

Appointments
An average patient will see Dr. Berkowitz for about 4-8 visits to resolve their issue. She encourages her patients to come to the office for monthly or bi-monthly well care visits to maintain health and vitality. 

Every appointment is a reserved time specifically for the patient. Dr. Berkowitz makes every attempt to keep a timely schedule. Please come five minutes early for your appointment and call twenty four hours in advance to cancel an appointment. If an appointment is cancelled or rescheduled with less than 24 hours notice, there will be a cancellation fee of $100.

Fees
Initial Visit: $200, 60 minutes.

Initial Extended Visit: $240, 60+ minutes.

Standard Visit: $110, 20 minutes.

Extended Visit: $130, 20-30 minutes.

Youth Visit: $70, 20 minutes.

Insurance
Patients pay for chiropractic services at the time of each visit. Receipts are given to the patient before leaving for insurance reimbursement. 

Payment Policy
Dr. Berkowitz accepts Venmo, cash, credit cards, checks, flex-spending, and health savings accounts. Dr. Berkowitz does not participate with any HMOs.
